Mill City Youth Football & Cheer Inc. has set up a program that will make registration
more pocket friendly. Space is limited! Teams fill up fast! Register early and begin
making monthly payments to ensure the registration costof $100.00 is paid by Aug.
1st. The payment chart below shows the amount
you could pay per month depending on the month you resgister. No child will
be turned away for their inability to pay the registration fee, however you must speak with Registration to discuss further.
We will be fielding 7 Football teams, on an age/ grade based system and playing in the largest Youth Football
& Cheerleading program in Lawrence.

Registration Form
Registration for football involves payment
& the following paperwork... - Copy of Participants Birth Certificate (if original is in a different language it must be translated
to English)
- Copy of
health form dated in current year or a letter from the doctor indicating the participant is healthly enough to participate
in football or cheerleading
- Copy of the participants
most recent report card showing grades for the full school year.
- Verification
of Address i.e. Copy of recent utility bill
